This is Kid MarsCat's digital restoration of the artwork for the cassette. It's not perfect but it's the closest to a proper scan she could do with limited resources and assets.

In the Chris Bieniek article about the Heiankyo Alien promotional cassette, it's mentioned that there was also an official t-shirt made for the game:
The real surprise is the last track, “Message From the Alien,” the lone cut on Side B of the cassette. It’s a professionally narrated and voice-acted recording that was apparently made for some type of Heiankyo Alien telephone hotline. “As a special reward to those who dare to battle with the evil from beyond,” it says, “we’ll give away a free Meldac original designer T-shirt to the first 80 warriors who buy Heiankyo Alien. These are one-of-a-kind T-shirts featuring the Heiankyo Alien in all his gruesome glory.”
I found an auction for a Heiankyo Alien t-shirt on the yahoo.co.jp auctions site (the original link is here, but it might not be there for long), which might be the t-shirt they were referring to on the tape.
